So long as incentive structures are unchanged, Overtreatment = Treatment = $$$.

The (mostly well-meaning) "search and destroy" mentality in medicine leads to little attention paid to extensive "collateral damage." Prostate cancer (e.g., http://www.cancer.gov/cancertopics/treatment/prostate/overtreatment0710 ) and breast cancer (e.g., http://www.medscape.com/viewarticle/584147 ) are just two examples.
