Bruce, I think you're wrong to take an all-or-nothing view of exchanging data. And you can get market advantage by sharing data. Patient shopping might be possible - but if patients can do it, so can providers. More here: http://www.healthintersections.com.au/?p=1616

Also, there are some who are more focused on overhead reduction than on old business models - they just want to share data.
